ongoing treatment, note pending tests or therapies with urgency, and document any necessary medication
substitutions. The information you provide will serve as the foundation for their care abroad, allowing their new
healthcare team to act promptly and appropriately from the moment they arrive.
Your collaboration in completing this checklist will greatly support your patient in maintaining their health and
